Please print out the Parental Consent Form and Medical Form, fill them out, sign them and mail them along with a copy of your most recent physical (within the past two years) and a money order of $200.00, made out to Randy Edsall Football Camp (Enrollment fee is non-refundable) to:

Per Connecticut State Law, any camper who brings medication with him or needs medication to be administered to him (including Tylenol, Epipen, inhaler, etc.) during camp must have his doctor complete the following form. Please make sure this form is filled out completely and return it to the Randy Edsall football camp with his physical.
