Parental Consent and Medical Forms are required for all campers.

The Parental Consent Form must be completed and signed by the camper's parent/guardian and mailed in with your initial payment, or within two weeks after registering and paying online.

The Medical Form must be filled out by the camper's physician and is due before the start of camp. Any physical taken within two years of the start of camp is valid and maybe submitted in place of our medical forms as long as the information is consistent with what our form requires. We cannot allow any camper to participate until we receive a copy of his physical.

IMPORTANT: Per Connecticut State Law, any camper who brings medication with him or needs medication to be administered to him (including Tylenol, Epipen, inhaler, etc.) during camp must have his doctor complete the following form. Please make sure this form is filled out completely and return it to the Randy Edsall football camp with his physical. AUTHORIZATION FOR THE ADMINISTRATION OF MEDICATIONS
